Kansas Frontier Behavioral Health Integration Collaborative (FBHIC)
ICHD has been awarded approximately $1.7 million through the Kansas Rural Health Transformation Program to launch the Kansas Frontier Behavioral Health Integration Collaborative (FBHIC), a regional partnership with five hospital partners serving Clark, Comanche, Edwards, and Kiowa counties. We are now recruiting for the founding team that will bring this initiative to life. All positions are full time and based in Greensburg, Kansas, with regional travel required.
Project Manager
The Project Manager is the central operational lead for the FBHIC regional partnership, responsible for day-to-day coordination across all five hospital partner sites, partner communication, fiscal compliance, and grant reporting. This is a full-time, grant-funded position reporting to the Executive Director. Salary: $80,000 annually.
Key Responsibilities:
- Serve as the primary operational lead for the FBHIC partnership, managing day-to-day project activities across all five hospital partner sites
- Facilitate communication and coordination among all project partners and serve as liaison to consultants, legal counsel, the external evaluator, and state program staff
- Oversee recruitment, onboarding, and supervision of FBHIC project staff
- Monitor project milestones and deliverables and ensure implementation aligns with the approved workplan and timeline
- Maintain compliant fiscal management of the FBHIC grant in accordance with Kansas RHTP requirements and 2 CFR 200 Uniform Guidance
- Prepare and submit progress reports, financial reports, and required documentation to KDHE and key project stakeholders
Minimum Qualifications:
- Bachelor’s degree in public health, healthcare administration, behavioral health, social work, nursing, business administration, or a related field required; master’s degree preferred
- Minimum of three years of experience in project management, program coordination, or a related role; experience in healthcare, behavioral health, or rural health settings strongly preferred
- Demonstrated experience managing grant-funded programs and maintaining compliance with federal or state funding requirements
- Strong written and verbal communication skills, including the ability to prepare professional reports and present to diverse audiences
- Valid Kansas driver’s license and reliable transportation; regional travel required

Care Coordination Lead
The Care Coordination Lead is the clinical and programmatic backbone of the FBHIC partnership’s care coordination infrastructure, responsible for identifying, developing, piloting, and documenting comprehensive care coordination pathways that connect primary care, emergency medicine, behavioral health providers, and community health workers across all five partner hospital sites. This is a full-time, grant-funded position reporting to the Project Manager. Salary: $60,000 to $75,000 annually.
Key Responsibilities:
- Develop, pilot, test, and document care coordination pathways connecting primary care, emergency departments, behavioral health providers, and community health workers across all FBHIC partner hospitals
- Design referral pathways and discharge follow-up protocols to ensure patients with behavioral health or substance use concerns receive timely follow-up care
- Serve as the primary programmatic liaison between ICHD clinical staff and hospital partner teams
- Support the pilot integration of behavioral health services into primary care, emergency department, outpatient, and specialty settings at partner hospitals
- Work closely with the Data and Reporting Specialist to define care coordination data collection requirements and contribute to shared performance dashboards
- Provide technical assistance to partner hospital staff on care coordination best practices and behavioral health integration workflows
Minimum Qualifications:
- Bachelor of Social Work (BSW), Master of Social Work (MSW), or Registered Nurse (RN) required; MSW or RN with care coordination experience strongly preferred
- Minimum of two years of experience in care coordination, case management, care navigation, or a closely related clinical or programmatic role
- Experience working in or with healthcare settings, including hospitals, primary care, emergency medicine, or integrated care environments
- Demonstrated ability to develop and document clinical workflows, referral processes, or care protocols

Crisis Case Manager (3 Positions Available)
The Crisis Case Manager provides crisis intervention, stabilization support, and community-based case management services to adults experiencing acute behavioral health crises and those in the period immediately following a crisis episode. This position works across community settings, ICHD facilities, emergency departments, and other points of contact to deliver timely, trauma-informed, recovery-oriented care. Three full-time positions are available. This position is funded through the Kansas Rural Health Transformation Program as part of the FBHIC and reports to the CSS Director. Salary: $55,000 annually.
Key Responsibilities:
- Respond to behavioral health crisis situations in the community, at ICHD facilities, and in coordination with emergency departments, law enforcement, and first responders
- Provide direct crisis intervention services including de-escalation, safety assessment, safety planning, and coordination of appropriate level of care
- Conduct risk assessments for individuals presenting with suicidal ideation, self-harm, psychiatric decompensation, or acute substance use crises and facilitate appropriate dispositions
- Carry an assigned caseload of individuals in the stabilization period following a crisis, providing follow-up outreach, service coordination, and support
- Collaborate with FBHIC partner hospitals and emergency departments to facilitate timely behavioral health crisis response, warm handoffs, and coordinated follow-up care
- Complete all service documentation accurately and within required timeframes in accordance with CCBHC certification requirements and ICHD policy
Minimum Qualifications:
- Bachelor’s degree in social work, psychology, human services, or a related field required; master’s degree preferred
- Experience in crisis intervention, behavioral health case management, or a related direct-service role preferred
- Familiarity with crisis response models, de-escalation techniques, and mental health first aid principles
- Strong interpersonal skills and the ability to build rapport quickly with individuals in acute distress
- Proficiency in Microsoft Office and ability to learn electronic health record systems

Community Health Worker (2 Positions Available)
The Community Health Worker (CHW) is a full-time, field-based position serving as a trusted bridge between patients and the healthcare system across Clark, Comanche, Edwards, and Kiowa counties. Two positions are available. These positions are funded through ICHD’s negotiated Prospective Payment System (PPS) rate and are designed to be sustainable beyond the initial grant period. This position reports to the Care Coordination Lead. Salary: $55,000 to $65,000 annually.
Key Responsibilities:
- Provide reliable transportation for patients to and from appointments at FBHIC partner hospitals, ICHD clinical sites, and other healthcare and community service locations across the four-county region
- Assist eligible individuals with Medicaid and Medicare benefit enrollment, including gathering required documentation, completing applications, and following up on enrollment status
- Conduct post-discharge outreach and follow-up for high-risk patients transitioning from hospital care back to home or community settings
- Connect patients to community resources including food assistance, housing support, utility assistance, and other social services that affect health outcomes
- Build and maintain trusted relationships with patients, families, and community members across the four-county service area
- Document all patient interactions, referrals, transportation activities, and follow-up accurately in ICHD’s electronic health record system
Minimum Qualifications:
- High school diploma or GED required; associate’s or bachelor’s degree in social work, human services, public health, or a related field preferred
- Current Kansas Community Health Worker (CHW) certification, or willingness and ability to obtain certification within the first six months of employment
- Demonstrated ability to build trusting relationships with individuals from diverse backgrounds, including those experiencing poverty, behavioral health challenges, chronic illness, or social isolation
- Valid Kansas driver’s license, clean driving record, and ability to operate ICHD vehicles safely across a large rural geographic area; extensive daily driving required
- Basic computer skills and ability to learn and use electronic health record systems

Therapist
The Iroquois Center for Human Development is seeking a dedicated and compassionate Therapist to provide high-quality mental health services to individuals, families, and groups across the lifespan. The Therapist will utilize evidence-based treatment modalities, including Cognitive Behavioral Therapy (CBT), to support clients in achieving their therapeutic goals.
Qualifications:
- Must hold a license from the Kansas Behavioral Sciences Regulatory Board (BSRB) at the intermediate or clinical level.
- Strong preference will be given to clinically licensed applicants.
- Intermediate licensed applicants must agree to pursue their clinical license and complete the process within two years of hire (clinical supervision provided).
- Training in trauma-informed, evidence-based practices such as Eye Movement Desensitization and Reprocessing (EMDR) is preferred. Additional preferred modalities include (list others).
Responsibilities:
- Provide individual, group, and family therapy to clients across the lifespan.
- Utilize CBT and other evidence-based treatment modalities to address a range of mental health concerns.
- Maintain a caseload that allows for 4 to 8 client contacts per day, totaling 5 to 6 hours of clinical work, depending on client acuity and family system needs.
- Collaborate with interdisciplinary teams to ensure comprehensive care and treatment planning.
- Maintain accurate and timely documentation in accordance with regulatory and organizational standards.
Preferred Skills & Experience:
- Experience working in a CCBHC or CMHC setting.
- Strong understanding of trauma-informed care and evidence-based interventions.
- Ability to work effectively with diverse populations and complex family systems.

The Iroquois Center for Human Development (ICHD) is seeking a compassionate and motivated Peer Support Specialist to join our team. This role is ideal for someone with lived experience in recovery who is passionate about helping others navigate their own recovery journey.
Position Overview: As a Peer Support Specialist, you will provide community-based support to individuals facing mental health and/or substance use challenges. You will use your personal recovery experience to inspire hope, build connections, and support clients in achieving their goals.

Transitional
The Transitional program is designed as a step between higher levels of care and independent living, for persons struggling with severe and persistent mental health symptoms. It is widely known across the state of Kansas, and we are proud of the lives that our staff have impacted through this program! We provide 24/7 staffing for individuals residing in this program, and our staff teach skills needed to return to community based independent living.
This position provides one-on-one support and/or supervision to patients with severe and persistent mental illnesses in the activities of daily living or in maintaining daily routines critical to a stable lifestyle. Provide training in daily living skills to improve self-help that impact living in the community. Monitor progress toward meeting treatment plan goals. Provide support and supervision during patient crises.

Minimum Qualifications:
- High School diploma or GED.
- Must have the ability to communicate positively and effectively with others, work independently as well as be a productive part of a team.
